Circulating Erythrocyte Insulin Receptor in adult Onset Diabetes Mellitus and Chronic Renal Failure

Abstract
For the purpose of clinical application of insulin receptor assay, we studied the specific binding of 125I-insulin. in vitro to circulating erythrocytes from 10 normal volunteers, 15 patients with non-obese diabetes mellitus and 15 patients with chronic renal failure. The specific 125I-insulin binding was measured by competitive binding of 1151-insulin and unlabeled insulin to insulin receptor of erythrocytes. The results were as follows:
1) The mean maximal percent specific 1251-insulin bindings of the normal subjects, the patients with diabetes rellitus and the patients with chronic renal failure were 6.61.9, 4.30.6 and 3.4 0.570, respectively.
2) The maximal insulin receptor concentrations calculated by Scatchard analysts were 0.90.2 and 0.40.1 ng/4.4X 1Gkells in the patients with diabetes mellitus and the patients with chronic renal failure, respectively, both being significantly lower than 2.76.8 ng/4.4X I00 cells in the ncrmal subjects (p<0.05) .
3) The mean number of receptcr sites per cells obtained by multiplying by Avogadro¢¥s number were 10. 11.5 in the patients with chronic renal failure and 22.44.3 in the patients with diabetes mellitus, both being significantly lower than 67.2 18.3 in the normal subjects (p<0. C5).
4) The mean erythrocyte receptcr affinity fcr insulin was 3.00.4 M`I X 1010 in the pat:ents with chronic renal failure, being s¢¥gnificar.tly higher than 1.90.5 and 2.00.2 M-1X 1010 in the ncrmal subjects and patients with diabetes rnellitus(p< 0.05).
5) The mean plasma insulin level was 39.9 15.3, 43.321.4 and 40.819.5 pU/ml in the ncrmal subjects, the patients with diabetes mellitus and the patients with chronic renal failure, respectively.
It is concluded that the low level ¢¥of insulin receptor concentration of the cell may be a contributory factor to glucose intclerarce or insulin resistance in the patients with diabetes mellitus and in the patients with chronic renal failure.
